Is the T16 exam open book?

Yes. ICC's Contractor/Trades exams are open book. You can bring approved references into the exam. The official ICC exam catalog lists which references are approved for the T16 specifically. Knowing your codebook cold matters more than memorization.

What happens if I don't pass?
